בצליאק, כמו בהרבה מחלות אחרות מדברים על מודל הקרחון.
החולים אותם אנו רואים ומאבחנים נכון הם לרוב החולים עם ההתייצגות הקלאסית, מעל ל 50% מחולי הצליאק הם עם התייצגות לא קלאסית, קרי, בביופסיה מהמעי הדק נראה השטחה של ווילי אך אין להם בכלל סימפטומים של מערכת העיכול. הם בד"כ ייתיצגו עם אנמיה ועוד. (יש איזו לימפומה שקשורה אליהם.
מתחתם נמצא בסיס עוד יותר רחב של חולים שיש להם פגיעה בווילי אך אין להם כל סימפטומים והם נמצאים בסיכון לסיבוכים ומתחתם יש את הקבוצה עם HLA המתאים (נדמה לי DRQ, אבל אני לא בטוח וזה לא חשוב)

בדיקות גנטיות ל DQ2 ו DQ8 יחד עם מציאת נוגדנים בסרום לאחר חשיפה לגלוטן ימצאו לך את כלל אוכלוסית החולים בפועל או בפוטנציה. מצד שני יש הרבה מאוד "רגישים לגלוטן" שהרגישות שלהם היא פסיכוסומטית, כפי שהמחקר שהבאתי מראה.

אם יש למשהו רגישות פסיכוסומטית לגלוטן, והימנעות מגולטן תמנע מהם את הסימפטומים אז אין סיבה שלא ימנעו מגלוטן.


מעבר לזה אין שום סיכוי בעולם שחברת ביטוח תאשר בדיקות של HLA באופן גורף לאוכלוסיה כדי לגלות מי כן ומי לא יש לו רגישות פוטנציאלית.
מצד שני ברור שרוב האנשים עם HLA שעושה אותם פגיעים ויאכלו גלוטן מהבוקר עד הערב וכלום לא יקרה להם.

בלחם לבן יש הרבה פחממות שנספגות מהר בדם ומעלות סיכון להשמנה וסכרת.
כך שהפחתת הלחם הלבן בדיאטה זה לא בהכרח רע.
